Transfusion shared care communication form

We have been made aware of a technical issue affecting the blood transfusion shared care communication form. The shared‑care blood transfusion communication form was developed and is owned by the National Blood Transfusion Committee (NBTC) Shared Care Working Group with input from SHOT. The issue is being investigated by relevant colleagues who are working to resolve the problem.

While this issue is being addressed, the form will not be available. Staff must not use this form until the issue is resolved.

The form is currently being refined and will be re‑released in the coming weeks. In the meantime, if you have any questions about the form, please contact NBTC Shared Care Working Group chair: katharine.maynard1@nhs.net
